Clinical Characteristics of Aspergilloma

Abstract
Purpose: The symptoms of pulmonary aspergilloma are various, from none to life-threatening massive hemoptysis. There were few clinical data about aspergilloma patients in Korea. This study was performed to investigate the clinical course of patients with pulmonary aspergilloma. Materials and Methods: Twenty patients with aspergilloma were enrolled. Their medical records were retrospectively reviewed for the clinical informations, including symptoms, underlying lung diseases, duration of underlying lung disease, location and size of the aspergilloma, treatments and clinical courses. Results: Eleven male and 9 female patients were enrolled. Their mean age was 56.2 ¡¾ 14.5 years. Hemoptysis (80%) was the most frequent symptom, followed by cough (55%) and sputum (40%). Small amount-hemoptysis was noticed in 13 (65%) patients and 5 (25%) patients had massive hemoptysis. The most common underlying lung disease was tuberculous destroyed lung (65%). Right upper lobe (40%) and left upper lobe (40%) were the most frequent locations of aspergilloma. The mean size of aspergilloma was 30.6 ¡¾ 16.0 mm. There was no relationship between the aspergilloma size and hemoptysis. The presence of underlying lung disease was associated with hemoptysis and the amount of hemoptysis. Medical treatment was usually enough for non-massive hemoptysis, and massive hemoptysis was required bronchial arterial embolization. Even the treatment, most patients with hemoptysis had suffered from intermittent hemoptysis for several years. Conclusion: The main symptom of pulmonary aspergilloma was hemoptysis. Tuberculous destroyed lung was the most common underlying lung disease. The presece of underlying lung disease was associated
with hemoptysis. Even with medical treatment and arterial embolization, most patients had suffered from hemoptysis for many years.
